About this project

"Brückenpflege", a care service from the Cancer Center Freiburg, which has existed since 1994, is now part of the Palliative Care Team "SPES" of the Specialized Outpatient Palliative Care (SAPV) at the Freiburg University Hospital. The SPES team can be seen in the photo. The "Brückenpflege" team itself consists of registered nurses who have additional training in oncology, specialized palliative care and pain therapy. The team plans the transition from the hospital to home with the patients and their relatives and accompanies them and their relatives there - if necessary and if they wish - until the end of their lives. Our offer in the greater Freiburg area If the disease is very advanced and requires complex care, we plan discharge from the clinic and prepare nursing and medical care at home. We work with clinics, general practitioners and specialists, nursing services and other healthcare institutions. We accompany our patients personally through a consistent contact person. We advise and guide patients, relatives and nursing staff. We support and advise on pain therapy. We take over the special treatment care. We provide support in the final phase of life. We are available around the clock in emergencies. In addition to funding, care service maintenance relies on donations in order to be able to maintain the service. The donations are used exclusively for the work and equipment of care service maintenance.
